Junior Protective Equipment EXEMPTION Form
This form is a notification by a parent or guardian that a junior (under 18 years) player has their consent to NOT wear mandatory protective equipment.
An explanation must be included in the response
Gumshields and shin pads are mandated equipment to wear under club rules for all junior members unless parental/guardian consent is provided.
